Q. What are your hours?
A. Monday to Thursday, 8:00am - 4:30pm and Friday, 8:00am - 1:00pm

Q. What other medical facilities are the Center for Spinal Disorder physicians affiliated with?                                                       
A. North Suburban Medical Center, Musculoskeletal Surgery Center, Platte Valley Medical Center, and St. Anthony Hospital North.

Q. What do I need to bring to my appointment?

Q. Can I fill out all the patient intake forms at home?
A. The Center for Spinal Disorders understands that time is of the essence and have provided all our forms for you to download online to make your appointment more efficient.

Q. What is a PA-C?

Q. When do I pay my co-payment (co-pay)?
A. Co-pays are determined by the respective insurance company.  It is the responsibility of the patient to pay his or her co-pay at the time of the appointment.
